This new NAC calculator gives a fast and accurate dose according to patient weight. May need to consult the Mathews-Rumack nomogram to asess if acetaminophen levels are toxic or not. If dose suspected to be above 140 mg/kg in a child or 7.5 gms in adult and time over 8 hours after ingestionon, administer NAC immediately. If below 8 hours and above 4 hours post acetaminophen ingestion do levels and compare with the nomogram (graph) that indicate if toxic or not. If below 4 hours post ingestion may proceed with decontamination (activated charcoal, ipecac?, lavage?) and do levels at 4 hours post ingestion.
